下腔靜脈過濾器是一靜脈阻隔器材,可「預防肺栓塞」。過濾器設計材質,提供過濾;並且可經 8.5 French 內徑導引鞘經皮置放,施行置放手術。

脈血栓形成(deep venous thrombosis,DVT)是血液脈內凝結導致脈迴流障礙性疾病,發生於下肢。栓子脱落可引起肺動脈栓塞(pulmonary embolism,PE),DVTPE統稱靜脈血栓栓塞症(venous thromboembolism,VTE),是同種疾病階段表現形式[1]。全球範圍內DVT和PE有高發病率,美國每年發60萬例VTE患者,發病率1‰~2‰,其中30%患者表現突發致死性PE[2-5]。亞洲國家VTE並見,我國例,診療水平提高及居民意識增加,其確診數上升,張偉[6]對北京醫院研究分析表明,1962—2009年1650例屍檢病例中PE檢出率為3.4%。PE突出高發病率(於心、腦血管疾病)和病死率(於腫瘤、心肌梗死)[7],已成導致院內患者非預期死亡主要疾病。通過VTE預防和及時診治可以程度減少致死性PE出現。對住院患者,是行動不便外科手術患者,明確急性VTE,首選抗凝藥物治療,可以避免栓子蔓延,同時促使人體自身纖溶機制消融產生血栓[8]。然而當急性VTE患者出現抗凝禁忌或抗凝治療失敗時,置入下腔靜脈濾器(inferior vena cava filters,IVCF)物理攔截下肢及盆腔靜脈系統血栓脱落成為預防PE手段[9-11]。

21世紀初IVCF使用數量迅速增加,IVCF相關的併發症如傾斜、移位、斷裂、血栓形成、下腔靜脈穿孔[12],使得置入IVCF爭議日益增多[13],多國內外研究團隊致力於新型IVCF研製避免上述事件發生。本研究IVCF臨牀應現狀及新型IVCF研究進展兩個方面綜合介紹了其發展歷史、分型和缺陷,並現在研究新型IVCF存在問題,提出解決構想,展望人體副作用、無需二次手術取出IVCF開發,早日投入臨牀使用。

型IVCF雖攔截血栓降低了致死性PE患病率,但置入人體後除非手術切開腔靜脈否則無法取出,患者需終生接受抗凝治療,同時置入濾器斷裂、移位、栓塞、症狀性下腔靜脈穿孔等出現率[16]。報道,濾器穿透下腔靜脈會導致胰腺炎、膿腫形成、十二指腸出血、主動脈損傷、輸尿管損傷和脊柱損傷[17-20],導致使型IVCF患者病死率沒有下降[21]。因此,既可攔截血栓、能時間內取出(研究分析顯示植入後29~54 d內取出獲益[22])可回收型IVCF成為臨牀應主流,型IVCF治療時代結束。

臨時型IVCF通過與外部導管和設備相接,於回收且下腔靜脈損傷,但其連接導管留置且外界相通,增加了感染機會,並且一項研究發現其療效並優於抗凝治療[23],臨牀應。目前美國食品藥品監督管理局(Food and Drug Administration,FDA)僅批准了一款臨時型IVCF(Angel Catheter)上市使用[24],適用於重症患者牀旁置入,同時可用於輸液。

基於心臟介入治療啓發,研究者們認為使用例如抗平滑肌增生、抗凝、抗炎藥物塗覆載藥下腔靜脈濾器(Drug-Eluting IVCF)會像藥物洗脱支架一樣降低血管局部炎性應答、阻止血管內膜過度增生,延長IVCF留置時間並便於回收,從而推動IVCF進一步發展。基質金屬蛋白酶(MMP)上述過程中起關鍵作用,研究[40-41]MMP抑制劑(MMPI)與聚乳酸-乙醇酸混合溶液噴塗可回收IVCF表面,通過IVCF支撐柱下腔靜脈壁接觸部位釋放MMPI,抑制MMP活性,從而抑制平滑肌細胞遷移,減少內膜增生,並動物模型體內證實其可增加管腔面積,預防血栓形成。研究濾器傳統血栓攔截功能基礎上取得了突破,Faltas[42]設想使用塗有循環腫瘤細胞表面標誌物異性抗體藥物洗脱IVCF吸引並裂解癌細胞,理論上減少腫瘤細胞通過靜脈系統血源性播散,這一説賦予了IVCF功能,但目前見進一步研究報道。

IVCF阻攔血栓功效依賴於結構設計,其結構影響下腔靜脈血流動力學。通過計算機模擬分析結構過濾單元以及支撐杆數目濾器性能影響,結構優化、血流動力學影響最小化新型IVCF研究提供了方法。Feng[52]使用有限元技術和計算流體力學探究了新型可轉換IVCF血管置入和變形過程中,濾器結構濾器變形行、血管壁應力分佈和濾器支撐度相互作用機理和影響,研究結果表明過濾器轉化支架之前,導致血管壁損傷,而轉換後濾器血管應力作用以及血流動力學影響有所減少,並動物模型體內驗證了其模擬過程性。楊彩紅[53]通過支撐杆數目8、10、12IVCF模型進行徑向支撐力測試,研究顯示其支持力隨支撐杆數目的添加而減小,擴張性能之相反。這一研究方法新型IVCF結構優化設計提供更科學參考理論。

IVCF留置體內引發一系列併發症受到國內外臨牀醫生廣泛關注,以至於美國胸科醫師學會(ACCP)發佈第10版VTE抗栓治療指南以及中華醫學會外科學分會血管外科學組制定第3版脈血栓形成診斷和治療指南中均提到推薦抗凝治療急性DVT或PE患者常規應用IVCF,但指南IVCF預防和減少PE發生作用予以了肯定,因此IVCF臨牀應關鍵於格把握其植入適應證。目前指南推薦適應證為:抗凝治療有禁忌或有併發症,或充分抗凝治療情況下發生PE者;適應證為:⑴髂、股靜脈或下腔靜脈內有漂浮血栓;⑵急性DVT擬行導管接觸性溶栓(CDT)、經皮機械性血栓清除術(PMT)或手術取栓血栓清除術者,具有急性DVT、PE危因素行腹部、盆腔或下肢手術患者。

減少濾器相關遠期併發症,臨牀應首選可回收型IVCF,但因其取出率、需要二次侵入性操作以及增加了患者經濟負擔缺陷飽受爭議。倡導推廣應用緻患者管理和臨牀隨訪來提高IVCF取出率基礎上,進一步研發出一款理想IVCF迫在眉睫,意義。綜上所述,我們認為新型IVCF應具備以下功能:可攔截血凝塊,防止新發或復發PE;VTE風險期結束後可遠程控制轉換或整體降解;濾器,保持良好徑向支撐性能同時損傷腔靜脈壁;具有生物相容性;對血流動力學影響;可影像學檢查中顯影於隨訪。相信材料學、組織工程領域深入研究發展,可早日研製出臨牀療效優異、併發症新型IVCF。
